Travel Fees are determined by distance in addition to the $5 notary cost per form mandated by Connecticut. Due to the popularity of weekend appointments, appointments must be booked in advance.

You must have two (2) forms of identification:

1: A government-issued photo ID with signature, such as a Driver’s License, Passport, etc.

2:An additional signed card, such as a credit card, library card, voter registration card, check-cashing card, etc. Social Security cards are not accepted.
